运动受伤:崴脚、冰敷、urgent care 与复出放行

美国运动受伤处理的核心说法是 RICE,顺序不能乱。 RICE 是 rest(休息)、ice(冰敷)、compression(加压包扎)、elevation(抬高)四个词的首字母。扭伤头 48 小时是冰敷,不是热敷——冰敷消肿止痛,热敷是 48 小时之后消肿了才用来放松僵硬的肌肉。冰敷的标准节奏是 20 minutes on, 20 minutes off,一次不要超过二十分钟,中间一定要隔开,直接拿冰袋贴皮肤太久会冻伤。这条在国内常被搞反,是这篇最值得记住的一条。
美国的就医分层跟国内很不一样,选错地方又贵又慢。 崴脚这种「不是要命但需要拍个片」的情况,美国人去的是 urgent care:不用预约(walk-in)、当天能看、能拍 X 光、比急诊便宜得多。ER(emergency room)是留给骨折、大出血、胸痛、昏迷这类真正紧急的情况,去 ER 排队几小时、账单可能是 urgent care 的好几倍。所以对话里朋友会说 That's urgent care, not the ER.——这不是省钱小气,是美国人的常识判断。
复出不是自己觉得好了就能上,要有人放行。 美国这套体系里,clear you to play(放行让你上场)是医生或理疗师给的,不是自己决定的,也不是队友说了算。流程通常是:拍片排除骨折 → 医生说这是 sprain(韧带拉伤)、告诉你护理方式 → 转给 physical therapy 做康复 → 力量和稳定性达标 → 医生签字放行 → 先 warm up、ease back into it、只打半场试试。很多人复出后还会戴一段时间的 ankle brace,这在美国球场上非常常见,没人会觉得你怂。

1. twisted my ankle 比 I hurt my foot 精确得多。
美国人说崴脚就是 I twisted my ankle 或者 I rolled my ankle,两句都常见。I broke my ankle 是骨折,不能随口说。别人问你 You good?,回一句 I twisted my ankle. 就把位置和性质都说清了。
2. 为什么头 48 小时只冰不热。
冰让血管收缩,能压住 swelling(肿胀)和 pain(疼痛);热让血管扩张,刚受伤时用热会让肿得更厉害。所以队友那句 Not today. Ice for the first two days. 不是随便说的,是标准处理。48 小时之后、肿消下去了,才轮到热敷来放松僵硬的肌肉。
3. walk it off 是好话,但不能用在扭伤上。
walk it off 意思是"走一走就好了",用在被撞了一下、手指戳了一下这种小伤上。韧带拉伤走不会好,只会更肿。这段话里你说了这句,队友立刻否掉——这是真实球场上会发生的对话。
4. sit this one out 是主动退出一局。
this one 指这一局,不是"今天不来了"。真要说今天全不打了,就说 I'm done for today. 或 I'm calling it.(我收工了)。这两个说法在美国球场上都很常用。

口语小注
1. urgent care 和 ER 的区别,美国人是从小被教的常识。
urgent care 处理"不致命但今天就得看"的问题:崴脚、发烧、耳朵疼、轻微割伤。ER 处理"再不去就出大事"的问题:骨折、大出血、胸痛、呼吸困难、昏迷。搞混的代价很实在——ER 排队几小时起,账单可能是 urgent care 的好几倍。
2. get it looked at 是去看病的标准口语说法。
不说 go to see a doctor 那么正式,就是 You should get it looked at.(你该去让人看看)。同类还有 get it checked out,两个都很常用,意思基本一样。
3. 钱的事一定要先问。
美国的诊所收费差别极大,Do they take my insurance?(收不收我的保险)和 How much is it gonna cost me?(得花多少钱)都是完全可以问、也应该问的问题。到前台先付的叫 copay(自付额)。Dave 那句 Call and ask first. 是过来人的建议。
4. I got you 不是"我抓到你了"。
在这段里它是"我帮你、我送你去"。口语中 I got you 还可以表示"我懂你意思了"或者"我来买单"。判断靠上下文,别按字面译。

口语小注
1. walk-in 就是"不用预约、直接进门"。
前台说 We're walk-in. 等于告诉你今天能看。urgent care 基本都是 walk-in,家庭医生通常要预约。这个词在美国医疗场景里出现频率极高,听懂它能省很多事。
2. rule out a fracture 是医生常用的说法。
意思是"先把骨折这个可能性排掉"。医生不先下结论,是因为脚踝扭伤和轻微骨折的症状很像,必须靠 X 光分。所以那句 I don't know yet. That's what the X-ray is for. 是负责任的说法,不是敷衍。
3. sprain 是韧带拉伤,不是骨折。
X 光只能看到骨头,看不到韧带,所以医生说 Nothing's broken. It's a sprain. 意思就是"骨头没事,问题在韧带"。严重程度一般分三度,一度最轻、三度是韧带完全撕裂。他没说度数,是因为医生通常先看恢复情况再判断。
4. 关于冰敷,医嘱和国内习惯不一样。

口语小注
1. 理疗师不会自己给你放行。

3. Not yet 比 No 好听,但意思是确定的。
Can I run on it? — Not yet. 这个回答在美国诊所里非常常见:不是永远不行,是现在不行。它后面通常接一个条件,比如 Pain-free walking first.(先做到走路不疼)。
4. 关于 brace,理疗师的比喻很地道。

1. clear you to play 是这套体系的关键词。
The doctor cleared me to play. 意思是"医生放行让我上场了"。clear 在这里是"批准、解除限制"。反过来问就是 You cleared?(你被放行了?)。在美国,队医、学校运动队、职业联盟都用这个词,含义很明确。

1. twisted my ankle 里 ankle 的重音在第一个音节。
