预约与改期

这一篇按从约到改到取消的顺序，拆成五个细化场景：打电话约新病人体检顺便问保险、约剪头发又临时改时间、约修车先说好估价和取车时间、约牙医洗牙确认共付额、餐厅订位临时取消被问要不要改期。

在美国，几乎所有服务都要预约：看医生、剪头发、修车、看牙、办证件、装网络，甚至退大件家电。跟中国最大的不同是——不预约直接上门（walk-in）经常会被拒，尤其是诊所和热门理发店。先把反复出现的词记住：

这个场景真正难的不是「约」这个动作，而是三件事。

第一，听懂对方报的时段。 Thursday the eighth at eight fifteen 一口气报出来，星期几、几号、几点全在里面，漏一个就约错。听到就复述一遍确认：So that's Thursday the eighth at 8:15, right?

第二，改约和取消是有成本的。 美国诊所、牙医普遍有 no-show fee（通常 25–50 美元）和 late cancellation fee，一般要求提前 24 小时通知；理发店常是 12–24 小时，热门店可能收一半费用；修车提前一天打电话基本免费；热门餐厅订位可能要押金，no-show 会扣。提前说一句 Something came up 通常一分钱不收，直接不去则可能留下记录、下次约不上。

第三，医疗场景有一堆保险术语，听不懂就多花钱。 copay 是每次就诊固定自付的钱；deductible 是一年内你要先自付满的金额，之后保险才开始付大头；in-network 指这位医生在你的保险网络内，价格便宜得多。约之前一定问一句 Are you in network?

另外提醒一句：美国的预约通常不是跟医生本人约，而是跟前台（receptionist）或调度员约。 对方会问你的姓名、出生日期、保险、电话；第一次去叫 new patient，要提前 15 分钟到，填一堆表格。新病人的第一个时段往往排得最远，急的话直接问 When's the earliest you have? 或者问 Do you have a cancellation list?（有人取消就通知你）。

1. physical 就是体检，别翻译成 body check。

美式说法：annual physical（年度体检）、checkup（一般检查）。physical exam 也对，但口语里直接说 physical 最常见：I'd like to set up a physical.

2. in network 是省钱的关键词。

• in-network：这家诊所在你的保险网络内，价格按合同走，便宜。
• out-of-network：网络外，可能贵好几倍，有些计划干脆不报。
• 约之前问 Are you in network?，并且把对方确认的答案记下来，最好让对方在系统里查你的具体计划。

3. copay 和 deductible 不是一回事。

• copay（共付额）：每次就诊固定自付的金额，比如 25 美元，当场付。
• deductible（免赔额）：一年内你要先自己付满的总额（常见 500–3000 美元），超过之后保险才开始付大头。
• 体检（preventive care）在 ACA 保险下通常全额覆盖、不走 deductible，但如果医生顺便处理了别的问题，那次就可能被算成 sick visit 而收费。这就是为什么前台要提醒你 If it turns into a sick visit, there may be a copay.

4. Do you have a cancellation list? 是插队神器。

新病人的时段往往排到几周后。问一句 Could you put me on the cancellation list?，有人临时取消就会打给你，经常两三天就能看上。

1. 改约要提前多久，直接关系到会不会被收费。

约的时候顺口问一句 What's your cancellation policy? 就清楚了。真的临时有事，越早打电话越好——提前一天说 Something came up 通常一分钱不收，直接不去则会留下记录。

2. 理发：just a trim 和「剪短一点」不是一回事。

中国学生最常踩的坑是说 Cut it short，结果被剪掉一大截。正确做法是给长度，不给形容词：

• Just a trim. —— 只修发尾
• Take about an inch off. —— 剪掉大约一英寸（2.5 厘米）
• A little shorter on the sides, leave the top alone. —— 两边短一点，上面别动
• 男士：Number two on the sides, scissors on top.（两边推二号刀头，上面用剪刀）——number + 数字 是电推剪的限位梳号数，数字越小越短

3. 理发师一定要给小费，15%–20%。

现金直接给最方便，刷卡时机器上也能选。这是美国少数几个「服务人员主要靠小费」的行业之一。不给小费不会被追着要，但下次约时间时你大概率会发现「没空位」。

4. 迟到了就先打电话。

I'm running about ten minutes late — is that okay? 这一句能救回你的预约。大多数理发店和诊所迟到 10 分钟以内都能接受（We can still take you.），迟到 20 分钟以上可能被要求改约。

标准流程是：描述问题 → 要估价（estimate）→ 他们打电话报价 → 你同意后才动手。修理厂说 We won't do anything without your okay 就是在承诺这一点。听到报价后你可以只说 Let me think about it 或者 I'll get back to you，不必当场答应。

2. diagnostic fee 通常可以抵维修费。

诊断费常见 100–180 美元。很多店规定「在这儿修就抵掉」，问一句 Does the diagnostic go toward the repair? 就能确认。不在这儿修，就只付诊断费把车开走——这叫 get a second opinion，美国人不觉得这是不信任。

3. 换机油要问清 synthetic 还是 conventional。

4. drop off 和 wait 是两种取车方式。

1. cleaning 和 deep cleaning 差的是几百美元。

• regular cleaning / prophylaxis：常规洗牙，在牙龈线以上，多数保险全额覆盖，通常和半年一次的检查一起做。
• deep cleaning / scaling and root planing：深度洁牙，在牙龈线以下，按**象限（quadrant）**收费，常见几百美元，可能要分两次做。
• 关键动作：先要 treatment plan（治疗方案）和费用估价，问 What's my out-of-pocket for this?（我自己要付多少）。美国牙科诊所费用差异极大，同一项目不同诊所可以差一倍以上。

2. no-show fee 和 late cancellation fee 是两笔不同的钱。

人不来又不提前说，叫 no-show，通常收全额或 50 美元；提前但不足 24 小时取消，叫 late cancellation，有些诊所收一半。提前 24 小时以上取消一般不收费，这就是那句 just give us twenty-four hours 的意思。

3. X-rays 一般一年有效。

4. fluoride、sealant 这类项目常常是额外收费的。

洁牙师在椅子上顺口问一句 Do you want fluoride today?，很多人下意识说 yes，结账才发现是自费。先问 Is that covered or extra? 再答应，不会显得小气。

1. 热门餐厅的订位政策这几年变严了。

疫情之后，很多美国热门餐厅开始对订位收押金（deposit）或者要求信用卡担保，no-show 直接扣钱（常见每人 25–75 美元）。人数多（8 人以上）常常自动加收小费，账单上会写 gratuity included。订位时问清 Is there a deposit? 和 What's your no-show policy?

2. 取消要趁早，改期比取消好。

前台问 Do you want to cancel, or move it? 是在给你台阶——改期对餐厅来说损失小得多，所以态度也更好。真去不了，提前一天打电话，通常什么都不会发生。

3. party of four 是「四位」，不是派对。

party 在餐厅语境里就是一桌客人的人数。进门说 Table for two，电话订位说 Party of four，两种说法都对。Party of six or more 常常会触发自动加小费（账单上会写 gratuity included）。

4. 迟到 15 分钟是餐厅的普遍底线。

绝大多数餐厅保留订位 15 分钟，超时就把桌子放给别人。要迟到就提前打电话：We're running about ten minutes behind — can you still hold it? 这一句经常能把位子保住。
