急诊中心（Urgent Care）：不用预约、登记分诊、缝针、扭伤拍片、拿医嘱

半夜孩子耳朵疼、切菜切到手、打球崴了脚——这些情况等不到下周的家庭医生，又没严重到要叫救护车，美国人的第一选择是 urgent care（急诊中心，也叫 walk-in clinic）。它的核心卖点就一句话：不用预约，走进去当天就能看。感冒发烧、耳朵发炎、尿路感染、扭伤、缝几针、拍个 X 光，这些都在它的业务范围里。费用比医院急诊（ER）便宜很多，copay 常见 $50–$75，而 ER 光是"进门费"就常常是四位数。

但它不是万能的，去了也可能被转走。 胸痛、呼吸困难、中风迹象、昏过去、大出血、骨头明显变形、需要 CT 或 MRI 的情况，urgent care 处理不了——前台或分诊护士会直接让你去 ER，或者当场打 911。判断标准不是"疼不疼"，而是"会不会马上没命"。

一次 urgent care 的流程是固定的：前台登记（保险卡 + 有照片的身份证件 + 说明来因）→ 候诊区等叫名字（表面上是先到先看，实际上分诊按严重程度插队，胸痛的人会直接排到最前面）→ 护士叫进去量血压体温、问病史、问 1 到 10 分的疼痛→ 医生看诊、检查 → 现场就能拍 X 光、缝针、开药 → 拿 discharge instructions（离院说明）走人。整个过程通常一到两小时。

1. walk-in 是一个词，不是"走进来"。

We're walk-in = 我们收没预约的病人。反过来问诊所有没有这个业务，就说 Do you take walk-ins?。primary care（家庭医生）一般不收 walk-in，所以这句话问之前先想清楚自己去的是哪一档。

2. fill out 是"填表"，不是 fill in。

美式口语里填表几乎永远说 fill out the form。fill in 更多是"补上空缺的那一格"，比如 Fill in your date of birth here.

3. hand 'em over = 把它们交过来。

'em 是 them 的口语缩写，美国人天天说。前台说 You can hand 'em over when you're done 就是"填完一起给我"，不用听清每个词，听懂 when you're done 就够了。

4. self-pay rate 是没保险的人的救命词。

不说这句，诊所会按原价（charge master 价）给你开账单，常常是保险价的三到五倍。说了这句，前台通常会给你一个固定的现金价。问一句不丢人，能省几百块。

5. 前台说等 45 分钟，可能等两小时，也可能 20 分钟就叫你。

因为排队顺序按严重程度动态调整。有人胸痛进来，你就会被往后推。这不是针对你，是分诊制度本身就这样。

分诊：量生命体征、说清怎么伤的

口语小注

1. vitals 是"生命体征"，不是"重要的东西"。

血压、体温、心率、血氧，合起来叫 vitals。护士说 I'm gonna get your vitals 就是把袖带绑上、体温计塞嘴里，你不用回答什么，配合就行。

2. I don't remember 是完全合格的回答。

破伤风这个问题，美国成年人一半答不上来。硬编一个年份反而危险——医生会照着你说的年份判断要不要补打。答不上来就说不知道，护士会写 unknown。

3. 保健品和维生素也要报。

医生问 Are you on any medications? 的时候，很多人只报处方药。但保健品会影响麻醉、影响凝血，比如鱼油、银杏、圣约翰草。一句 Just a multivitamin 就够了，不报才是风险。

4. rolled my ankle 就是"崴了脚"。

不要说 I twisted my foot（听起来像故意扭的），也不要说 my ankle is broken（那是你自己下诊断）。I rolled my ankle 是美国人最常用的说法。

5. 分诊护士问的每一句都是筛危险的。

Any numbness or tingling? 查神经，When was your last tetanus shot? 查感染风险，Any allergies? 查用药安全。这些不是走流程，是决定医生接下来做什么，所以答得越具体越好。

2. 破伤风的时间线只有两条。

脏伤口（生肉、泥土、生锈金属、动物咬伤）——上次打超过 5 年就补一针。干净的小伤口——超过 10 年才补。所以医生说 more than five years ago 加 a dirty wound，是两条都占了。

3. irrigate 不是"消毒"，是"大量盐水冲洗"。

缝之前一定要冲，目的是把细菌和碎屑冲出来。这也是为什么别在家自己缝——冲不干净，感染率完全不是一个量级。

4. 感染征兆只有五个词，背下来。

1. 冰敷是"20 分钟敷、20 分钟停"，不是一直敷。

一直冰会让局部血流太差，反而拖慢恢复。前 48 小时尽量多做，中间隔一层薄毛巾，别把冰袋直接压在皮肤上。

2. RICE 的 C 是 compression，不是"包扎紧一点"。

用弹性绷带（ace bandage）从脚趾往小腿方向缠，不要缠到发麻或者变色。麻了、紫了、更疼了，就是缠太紧，松开重来。

3. 医生开的剂量是"400 到 600 毫克"，不是"两片"。

美国的布洛芬药片有 200 毫克一片的，也有 800 毫克一片的处方版。认毫克数，不要认片数——Two of the two-hundred-milligram tablets 才是准确说法。

4. follow up with your own doctor 不是敷衍。

urgent care 的定位就是"解决今天的问题"，不跟踪长期恢复。一周还肿、还疼，就该回家庭医生那里，可能要做进一步影像或者转骨科。

5. X 光的账单一定是单独来的。

医生当场说这句，是为了让你之后收到第二张账单别慌。影像费走的是另一条账，常常直接算进免赔额，也就是说那部分要你先自己付满。
