营养与饮食:看营养师、减重计划、食物过敏与忌口、外食怎么选

在美国管营养的正规头衔是 registered dietitian(RD / RDN,注册营养师):要学位、要临床实习、要执照考试。nutritionist(“营养师”)在很多州没有统一门槛,谁都能这么叫。所以找人之前问一句 Are you a registered dietitian? 是最省事的筛子——没人会觉得被冒犯,营养师自己也会先说明。
看营养师一般要医生转诊。医学营养治疗(medical nutrition therapy,MNT)在有糖尿病、糖尿病前期、肾病这类诊断的时候保险通常覆盖,一般一年 3–6 次。营养师办公室会先 verify benefits(向保险公司核实你能报几次),你打电话约的时候就可以顺便问一句。
第一次去通常做三件事:24 小时饮食回顾(Walk me through what you ate yesterday.)、量体重和腰围、算 BMI,然后一起定一个具体目标。这一步的重点是诚实——营养师要看你平常怎么吃,不是你为了好看临时改出来的那一周。所以医生会特意说 Don't change anything to impress me.
减重的硬事实只有这样几条:健康速度是每周 1–2 磅;减掉体重的 5%–10% 就已经能改善血糖和血压,不用减到理想体重才有收益;靠的是每天几百卡的热量缺口,不是断食速成。下面这张表是全篇反复出现的几个抓手。

- 乳糖不耐(lactose intolerance)不是牛奶过敏。 过敏是免疫系统的事,会起疹子、肿、呼吸困难,可能致命;乳糖不耐是肠道缺乳糖酶,表现是腹胀、肚子绞痛、拉肚子。所以不用戒奶——试 Lactaid 牛奶或者乳糖酶片,硬奶酪和酸奶通常没问题。
- 乳糜泻(celiac disease)不是"少吃点麸质"。 小麦、大麦、黑麦都不能碰,要严格无麸质。普通酱油是用小麦做的,要换 tamari;外食要问油炸锅和酱料,燕麦要选标了 certified gluten-free 的。
- 含糖饮料是最容易砍的一块。 一杯甜茶大约 200 卡,液体热量不顶饱,砍掉它往往比少吃一碗饭更有效。

口语小注
1. dietitian 和 nutritionist 在美国不是一回事。
RD / RDN 有执照和临床实习,医院、保险、诊所认的是这个;nutritionist 在很多州不受监管。约人之前问一句身份,几秒钟的事。
2. I'd like to lose about twenty pounds 比 I want to be thin 有用得多。

口语小注

口语小注
1. That's the easiest win 是"最容易见效的一步"。
营养师不会让你一次改十件事,而是先找性价比最高的那一个。含糖饮料通常就是它——一杯甜茶约 200 卡,砍掉不饿肚子。
2. non-starchy vegetables 指不含淀粉的菜。
绿叶菜、西兰花、黄瓜、番茄都算;土豆、玉米、南瓜这类含淀粉的算主食那四分之一,不算菜。搞混了盘子法就失效。
3. 主食不用戒,只是减半或者换粗粮。

Megan: An allergy is your immune system, and it can be dangerous — hives, swelling, trouble breathing. Intolerance is your gut not making enough lactase, the enzyme that digests milk sugar. Uncomfortable, but not dangerous.

1. 描述肚子的问题,cramping 和 bloating 是两个必会的词。
