在美国,心理问题第一站通常是家庭医生,不是心理诊所。年度体检也好,随便看个什么毛病也好,医生都会顺手做一次情绪筛查,问的往往是原话:Over the last two weeks, how often have you felt down, depressed, or hopeless? 紧接着还有一句几乎人人都被问到的 In the last two weeks, have you had any thoughts of hurting yourself?——这不是针对你,是标准流程,医生对每个病人都问一遍。答没有就过去了,不用觉得被冒犯,更不用慌。

真正做谈话治疗的人分三种,搞混了会白跑一趟:

多数人先看 therapist。 只有需要药物、或者药物要调整的时候,才会转到 psychiatrist;家庭医生本身也能开最常用那几种抗抑郁药。

保密(confidentiality)是这一行的地基,但它不是绝对的。 你说的话不会告诉你家人、不会告诉你公司、也不会随手进普通病历——除非你签授权。例外只有几种:你有伤害自己或他人的风险、涉及儿童虐待或老人虐待。第一次见面咨询师会主动把这几条讲给你听,这不是不信任你,是执业要求。

钱的部分:保险一般每次收一个 copay,常见 $20–$50;很多公司的 EAP(Employee Assistance Program)前 3–8 次是免费的,而且完全保密,主管看不到;在校学生的 counseling center 一般也免费。第一次叫 intake(初诊访谈),比后面几次长,主要是问背景、定目标。约的时候直接问 Do you take my insurance?、How much is a session?、Do you have evening openings? 就行。

跟家庭医生说出口:睡不着、集中不了、什么都提不起劲

Doctor: So, what brings you in today?

You: Honestly? I’ve been really stressed lately. I’m not sleeping and I can’t focus at work.

口语小注

1. I'm stressed 比 I'm depressed 好开口,医生也听得懂。

很多人第一次进诊室说不出"我抑郁",那就从压力说起。I've been really stressed、I can't sleep、I snap at everything——这三句说出来,医生基本就知道往哪个方向问了。别硬憋一个"专业词"。

2. 筛查问题答得越具体,越省事。

How often? 不是让你算精确天数,是要一个量级。Most days / More days than not / Maybe twice a week 都比 sometimes 有用。答得含糊,医生只能再问一轮。

3. 医生问"有没有伤害自己的念头",是每个人都问的。

这句在美国属于常规情绪筛查的固定一环,跟量血压一样。答 No 就过去了,不用解释一大堆;真有这个念头,直说反而是最快得到帮助的方式,医生不会因此把你怎么样,也不会通知你家人。

4. Would you be open to that? 是在征求同意,不是下命令。

美国医生提转介一般都用这种问法。你可以答应,也可以说 Can I think about it?、I'd rather try something else first.。没有同意,他不会直接替你约。

5. EAP 是最容易被忽略的免费资源。

Your company's EAP covers the first few sessions for free——这句话值好几百美元,而且完全保密,主管看不到。很多人做了几年都不知道自己公司有。打电话问人事或者翻福利页面,两个词就够:EAP 和 counseling。

口语小注

1. 咨询师叫 clients,不叫 patients。

这是这一行的习惯,跟医院不一样。所以约咨询时说 Are you taking new clients?,说 patients 也不算错,但听着像医院。前台如果回你 We're taking new clients,就是在说还收人。

2. intake 那次会问一堆背景问题,不代表你要"交代"。

初诊访谈会问到家庭、工作、睡眠、有没有看过心理科。这些不是审问,是建档。哪一项不想说,直接说 I'd rather not get into that yet. 完全可以,咨询师不会追着问。

3. 保密例外只有几种,第一次就会主动告诉你。**

If I believe you're in danger of hurting yourself or someone else, or if there's abuse of a child or an elderly person.——这句话背下来都值得。它的意思正好是反过来的:除了这几种,连你家人也不会被告知,除非你签授权。

4. 想让咨询师和家庭医生通气,要签 release。

Do you ever talk to my regular doctor? 这个问题问得好。默认答案是不会。要让两边协调,得签一份授权(release),而且你可以指定只分享哪一部分。

5. 约不到六点以后,也可以问 Do you have anything early?

很多人只问晚上,其实早班号也有人取消。另外一周一次的频率是起点,不是规定,经济或时间紧张,直接说 Can we do every other week?,咨询师一般都能安排。

谈焦虑:胸口发紧、心跳加快,是不是心脏病

口语小注

1. 焦虑和惊恐发作不是一回事,医生会分开问。

一直担心(I'm always worried about something)是焦虑;毫无来由突然发作、心跳飙、胸口紧、十分钟到半小时(it comes out of nowhere)是 panic attack。这两个词说准了,医生的判断会快很多。

2. 惊恐发作很不舒服,但不会致命。

It's your body's alarm system going off when there's no fire.——这句值得记。身体的警报系统误触发了,心跳、呼吸、出汗全是真反应,只是没有真的危险。知道这一点本身就能减轻一大半恐惧。

3. 呼吸练习的重点是"呼气比吸气长"。

In for four, out for six.——不是深呼吸,是慢呼气。吸气四拍、呼气六拍,做几分钟。真发作的时候不用做得标准,能慢下来一点就有效。

4. 5-4-3-2-1 是把注意力从脑子里拉回身体。

看五样、摸四样、听三种、闻两样、尝一样。听着像小孩游戏,但它逼你用感官占用注意力,比在心里劝自己"别想了"管用得多。

5. Am I having a heart attack? 一定要问出来。

这个问题不丢人,而且必须先排除心脏问题——这正是家庭医生做心电图的原因。排除了,才轮到心理层面处理。没查过心脏就别自己下结论。

口语小注

1. 说失眠要分三段讲:入睡、夜醒、早醒。

It takes me an hour to fall asleep(入睡难)、I wake up at three(半夜醒)、I lie there till the alarm(醒了睡不着)。这三段对应的问题不一样,医生会分开处理,所以别只说一句 I can't sleep。

2. 酒精是"帮你睡着、毁你后半夜"。

Alcohol helps you fall asleep and then wrecks the second half of the night.——这句是整幕里最值钱的一句。喝一杯确实更快入睡,但后半夜会变浅、变碎,三点醒就是这么来的。很多人以为酒是助眠的,正好相反。

3. 咖啡因的账要算到下午那罐可乐上。

很多人只算咖啡。Half the caffeine is still in you six hours later.——下午三点那杯,晚上九点还有一半。中午之后就别再喝了,这一条改善睡眠最快。

4. CBT-I 是失眠的一线疗法,不是"随便聊聊"。

口语小注

1. 抗抑郁药不是"吃了马上就舒服"。

Four to six weeks to feel the full effect.——四到六周。头两周往往只有副作用、没有效果,很多人就是这时候放弃的。知道这个时间表,才熬得过去。

2. 不能自己突然停,这是硬规矩。

发音与语调提示

1. anxiety 的重音在第二音节,anxious 在第一音节。

anxiety 读 /æŋˈzaɪəti/,an-ZY-e-ty;anxious 读 /ˈæŋkʃəs/,ANK-shus。这两个词经常连着出现,念错了对方要多反应半秒,慢慢说清楚就好。

2. therapist 的重音也在第一音节。
