眼科：验光配镜、隐形眼镜、眼睛干涩、眼底检查、眼镜店取镜

美国看眼睛分两种人：optometrist（OD，验光师） 负责验光、配镜和常见眼病；ophthalmologist（MD，眼科医生） 看眼病、做手术。常规验光配镜找 OD 就够了，需要手术或者复杂眼病，才会转到 MD 那边去。打电话约的时候说 I'd like to schedule an eye exam 就行，诊所自己会安排。

视力保险（vision plan，如 VSP、EyeMed）和医疗保险是分开的两套，这是最容易搞混、也最容易多花钱的地方。视力保险管"看清楚"这件事，医疗险管"眼睛生病"这件事：

处方是你的，可以拿走。 美国 FTC 的 Eyeglass Rule 规定，验光之后必须免费给你一份处方，你可以拿着它去任何一家配镜。直接说 Can I get a copy of my prescription? 就行——验光师不会为难你。隐形眼镜是另一份处方，要单独做一次 contact lens fitting（验配），通常一年有效，过期了要重新验配。

散瞳眼底检查（dilated eye exam）是另一件要提前知道的事：滴了扩瞳药水之后，你会模糊四到六小时，怕光，所以要带墨镜，别打算自己开车。糖尿病患者需要每年做一次，这一次走的是医疗保险，不是视力保险。另外记住几个危险信号：突然出现大量飞蚊、看到闪光、或者像有帘子挡住视线——当天就打电话或者去急诊，不要等。

回答时说清是哪一家、怎么来的：VSP, through my employer. 如果你只有医疗险没有视力保险，就直接说 I don't have vision insurance — do you have a self-pay rate?

3. toward frames 的意思是"这笔钱只能用在镜架上"。

1. Better one or two? 会问很多遍，答错没关系。

这是验光里最核心的环节，医生靠你的主观判断确定度数。拿不准就说 Can you do that again?，或者 They're really close——真实回答比硬选一个有用。全程大概十几轮，别烦。

2. 你有权拿到处方。

美国 FTC 的 Eyeglass Rule 规定，验光结束后必须免费给你一份处方，你可以拿着它去任何地方配镜。所以 Can I get a copy of my prescription? 这句话完全正当，验光师不会不高兴，反而会说 It's yours.

医生会说 Not dangerous, but it's not nothing——不管它，长期会伤到眼球表面。大多数人的干眼来自屏幕和隐形眼镜，改习惯比吃药管用。

2. 人工泪液一定要买无防腐剂的。

preservative-free 是关键，一天用超过四次的时候尤其重要。普通瓶装人工泪液里的防腐剂本身会刺激眼睛，一天最多四次；无防腐剂的小支装可以按需用。

3. 20-20-20 法则是最省钱的治疗。

每 20 分钟，看 20 英尺（约 6 米）外的东西 20 秒。加湿器、热敷、眨眼练习惯都能帮上忙。最严重的病人，医生才会提 punctal plugs（泪点栓）。

4. 糖尿病患者的眼底检查走医疗险，不是视力保险。

1. one or two 那一组数字要听清、答清。

Better one, or two? 是验光的核心问法，说 One. 的时候把元音拉满、别含糊，不然验光师会重复问。答不上来就说 Can you do that again?，重复问不丢人，配错度数才麻烦。

2. OD 和 OS 别念字母，念单词也行。

写处方上是 OD（右眼）/ OS（左眼），口头说的时候美国人一般直接说 right eye、left eye。听懂 OD is your right eye 这句就够用了。

3. tear 有两个读音，两个意思。

眼泪的 tear 读 /tɛr/，复数是 tears /tɛrz/；撕破的 tear 读 /tɛr/ 也一样，但 wear /wɛr/ 和 where 同音，说 I wear contacts 的时候别紧张，对方听的是句子不是单词。

4. dry 和 tired 的重音都在前面。
