在美国,报警、火警、急救统一打 911,不用记不同号码。这一篇练的就是那通电话——它可能是你一生中唯一一次打 911,但真打的时候,你的英语会瞬间退回最差的状态,所以每一句都值得练到不用想。

接线员(dispatcher)的开场顺序永远是固定的:

  1. 911, what's your emergency? —— 什么事
  2. What's your address? —— 地址,第一优先
  3. Is he conscious? Is he breathing? —— 人清不清醒、有没有呼吸
  4. Stay on the line. —— 别挂电话

地址说不清,救护车就找不到人,这是最要命的一环。提前把自己家地址背熟,并且练会怎么说:门牌号 → 街道名 → 公寓号 → 城市。如果说不出来,就找一个你能看到的标志物(I'm at the corner of Main and 5th),或者把电话给旁边英语好的人。

什么时候打 911、什么时候去 urgent care、什么时候不用去医院,这三个边界必须分清:

ER 和 urgent care 是两回事,别走错。 ER(Emergency Room)是医院急诊,24 小时开门,按严重程度排队,不是先来后到;费用极高,一次几百到几千美元起。Urgent care 是独立急诊中心,不用预约、当天能看,能处理扭伤、缝针、发烧、耳朵发炎这类问题,价格通常只有 ER 的零头。判断标准很简单:会不会没命。会,去 ER;不会,去 urgent care。

救护车(ambulance)在美国非常贵,通常 $1,000–$2,000 起,而且保险未必全报。所以判断标准是:危及生命就叫,不危及生命就自己开车或打车去 ER。这条不是省钱建议,是真实的美国生活常识。

门牌号 → 街道名 → 公寓号 → 城市

  • 1420 Maple Street, apartment 3B, Riverside. —— 一次报完,接线员一次就能录入
  • 看不出门牌号就报交叉路口:I'm at the corner of Main and 5th.
  • 在高速上就报方向和最近的出口:I'm on I-90 westbound, just past exit 12.
  • 实在说不清就报能看到的店:I'm in the CVS parking lot on Grand Avenue.

报错地址比报不出来更糟,所以不确定时宁可说 I'm not sure of the exact address, but I'm right next to…

口语小注

1. 接线员问的是 What's your location?,不是 Where are you?

两个都对,但前者是 911 的标准问法。回答顺序是固定的:门牌号 → 街道 → 公寓号 → 城市。1420 Maple Street, apartment 3B 这样报,接线员一次就能录入。

2. 别挂电话,接线员可能正在救你。

Stay on the line. 意思是"别挂"。911 接线员会在电话里教你怎么止血、怎么做心肺复苏(CPR)、怎么判断有没有呼吸。挂了电话就没人指导你了。

3. shaken up 是"吓着了",不是"被摇了"。

被撞的人、目击者、司机都可能 shaken up。这个词在事故现场出现频率极高,说 She's just really shaken up 就是"她就是吓得不轻"。

4. Don't move him 是真的别动。

怀疑颈椎、脊椎受伤时,搬动可能造成二次伤害,甚至导致瘫痪。所以接线员说别动就别动,哪怕你觉得他躺的姿势很别扭。唯一例外是现场有火灾、爆炸这类直接危险,那时才需要拖拽转移。

5. 救护车真的贵,但该叫还得叫。

非紧急情况自己开车或打车去 ER 是很多美国人的实际选择。但如果出现胸痛、呼吸困难、说不出话、意识模糊、大出血,别犹豫,叫车——这些情况你自己开车可能半路就失去意识了。钱的事后面有账单申诉和财务援助可以走。

在急诊室陪朋友,跟分诊护士说明情况

急诊室的真实流程

口语小注

1. 急诊的等待时间由分诊护士决定,不是先来后到。

一进门先见 triage nurse,他给你定一个紧急等级。胸痛的人会比崴脚的人先看到医生,哪怕崴脚的人先到两小时。这是制度设计,不是插队,别去质问护士。

2. 情况变差要主动说,这是最有用的一句话。

He's getting worse 或者 He feels like he might pass out——说出来等级就会被重新评估。上面那段对话里,正是因为陪同的人提了一句头晕,病人才直接进了诊室而不是候诊区。沉默等待是最吃亏的。

3. 没带保险卡不影响治疗。

美国法律(EMTALA)规定急诊必须先救治、后谈钱,不管有没有保险、有没有身份。所以 He doesn't have his insurance card with him 的答案是"没关系,之后补"。但之后一定要补,不然账单会按全价自费算。

4. He's stable 是好消息。

在医院你会反复听到 He's stable.——意思是生命体征稳定了,暂时没有生命危险。反过来,critical 或 in critical condition 是危重。这两个词听懂能省掉很多焦虑。

5. tetanus shot 是破伤风针。

被金属割伤、被动物咬伤、伤口有泥土时,医生几乎一定会问。When was your last tetanus shot? 是标准问题。答不上来就说 I don't remember,医生会直接给你补一针。

口语小注

1. I'm not sure 在笔录里是最安全的回答。

警察明确鼓励你说不确定。I'm not sure 不会让记录变差,但为了让回答听起来完整而编造会——目击记录要进正式报告,甚至可能上法庭,编的内容会造成严重后果。

2. 照片拍得越早越有用。

救护车来之前、车辆还没挪动的时候,拍:车辆位置、自行车位置、路面痕迹、双方车牌、红绿灯状态。这些是客观证据,比任何描述都硬。

3. called it in 是"报警"的口语说法。

Are you the one who called this in? 就是"是您报的警吗"。类似说法还有 You're the reporting party?,听到都是同一个意思。

4. 报案号(report number / case number)一定要拿。

它是之后查进度、找保险公司理赔的唯一凭据。当场记在手机里,别指望事后想起来。

5. 目击者有义务留下,但没有义务"下结论"。

留下来做笔录是公民义务,但判断谁的责任是警察和保险的事。你只需要说清你看到的动作和位置,不需要说 It was her fault.——那句话说出来反而可能被对方律师抓住。

不会英语:在医院申请口译

1. 口译是权利,不是人情。

美国联邦法律要求接受联邦资金的医院必须为有限英语能力的病人提供免费语言服务。所以 I need an interpreter who speaks Chinese. 这句话说出来,医院必须安排,不是帮忙。不要因为不好意思而硬撑——医疗信息传错的代价太高。

2. 电话口译是标准做法,很常见。

大部分医院用的是电话或视频口译服务(常见的有语言服务专线),接通通常只要一两分钟。听说是电话翻译不要觉得是敷衍,这是正规流程,专业口译员受过医学术语训练。

3. 不要让孩子当翻译。

这是美国医院明确反对的做法。孩子(尤其是未成年子女)翻译医疗信息,既容易出错,也给孩子造成心理负担。直接要求医院提供专业口译员。

4. 翻译在的时候,直接对医生说话。

不用看着翻译说"你告诉他……"。看着医生说话,让翻译去转述,这样沟通更顺,也不会漏掉信息。这是口译的基本礼仪。

5. 药名不知道就说不知道。

I don't know the name — the bottle is at home. 完全没问题。比乱说一个药名安全得多。更好的做法是手机里存一张药瓶照片,或者把药瓶随身带。

口语小注

1. discharge papers 是出院说明,一定拿好。

上面写着诊断、用药、复诊时间、危险信号、医生联系方式。看不懂可以要求翻译,拍照存手机——纸质的东西很容易丢。

2. stitches 是缝线。

I got eight stitches.(我缝了八针)、When do the stitches come out?(什么时候拆线)都是高频说法。十天左右拆线是常见安排,但不同部位不一样,以医生说的为准。

3. 感染信号要能听懂,而且要会自己说。

redness(发红)、swelling(肿胀)、pus(脓)、red streaks(红线)、fever(发烧)。其中"红线顺着胳膊往上走"是最严重的,说明感染已经进淋巴管,必须当天处理。

4. 美国急诊账单一定是分开的。

一次急诊可能收到:医院场地费(facility fee)、急诊医生费(physician fee)、影像费(radiology)、化验费(lab)——来自不同机构,几周内分别寄到。收到第一张别以为结清了。

5. 没带保险卡不影响治疗,但必须补。

发音与语调提示

1. 911 读作 “nine-one-one”,不读 “nine eleven”。

这是硬规则。接线员和所有美国人说的都是 nine-one-one。说成 nine eleven 虽然能听懂,但一听就知道不熟。
