在美国,高血压和糖尿病这类慢性病不住院治,是由 primary care(家庭医生)长期管的:确诊之后每隔三到六个月复诊一次,叫 follow-up visit。这类复诊最不一样的地方是——医生看的不是你今天舒不舒服,而是数字的趋势。所以去之前把家里的记录带上、把药瓶带上,比任何形容都管用。

这一篇按一次慢病管理的真实流程拆成五个场景:高血压复诊带家庭血压记录、糖尿病复诊看 A1c 和查脚、血糖掉到 62 那天的处理、跟糖尿病教育师谈怎么吃、续长期处方和预授权。

高血压最坑的一点是它没感觉。 血压 150 的时候人一样能上班、能睡、能打球,所以医生不会靠问你"感觉怎么样"来判断药有没有效,他看数字。你自己觉得挺好就把药停了,血压照样往上走,伤的是心、脑、肾。医生原话是:Don't stop it even if you feel fine — you feel fine because the medicine is working.

糖尿病管的是三件事:药、吃、查。 药是 metformin 打底,必须随餐吃,不然胃很难受;吃看的是盘子法和含糖饮料;查是每三个月看一次 A1c、每年查一次眼底、查一次脚、查一次尿(看肾)。这三件事缺一件,A1c 就下不来。

1. 医生说"没感觉"不是安慰你,是事实。

高血压被叫做 silent killer,就是因为血压高到 160 人也不一定有感觉。所以医生问的永远是数字,不是你今天的状态。复诊前把家庭记录整理好,比背一堆形容词有用。

2. 漏服要老实说,说了才有用。

I miss maybe one a week 听起来像在认错,但这是医生调方案的关键信息:一周漏一次,可能是给药方式的问题(比如换成一天一次的长效药),不一定是你不认真。答 Every day 反而让医生在错误的方向上加量。

3. 自己量血压有三个硬要求。

同一时间、坐五分钟、手臂齐心。这三条不满足,量出来的数字能差十个点,医生会照着这个假数字给你加药。带血压计去诊所对一次,是很容易被忽略但很值的一步。

4. Don't stop it on your own 是美国医生对慢病病人说得最多的一句。

5. dizzy 和 lightheaded 不是一回事。

口语小注

1. A1c 是糖尿病管理的总分数,别只盯着某一天。

它反映的是过去两三个月的平均血糖,所以见医生前熬夜、节食都改不了当天的 A1c。7.8 到 7.2 是实打实的进步,医生会明确说出来——听到这种话要接住,别只顾着说"还是高"。

2. 家庭血糖记录要说"空腹"和"饭后两小时"两个数。

只说"我平时一百四"医生没法判断是空腹高还是饭后高,这是两种完全不同的处理。固定两个时间点,各记一个数,一行一天,带去复诊。

3. 68 就算低血糖,哪怕你自己走回家了。

口语小注

1. 15 克规则的关键在"等"和"复测"。

很多人一慌就猛吃,血糖从 62 冲到 250,然后又掉。正确的是:固定 15 克 → 等 15 分钟 → 复测,不够再来一轮。整个过程要有耐心,不能"觉得还难受就再吃点"。

2. 葡萄糖片要"随身",不是"在家有"。

放车里、放办公室抽屉里、放床头,都等于没有——夏天车里还会化。口袋和随身包里各放一份,是护士会反复强调的一条。

3. 低血糖的症状要说具体的。

4. CGM 和扎手指不冲突,但确实省事。

1. 盘子法不需要称重,这是它最大的优点。

一个盘子分四份:一半非淀粉类蔬菜(青菜、西兰花、黄瓜、番茄),四分之一蛋白(肉、鱼、蛋、豆腐),四分之一主食(米饭、面、馒头、土豆)。中餐容易出问题的地方是主食占了大半个盘子。

2. 粥比白米饭升糖还快。

这是很多中国病人最意外的一条。米煮得越烂,吸收越快,所以白粥、稀饭、米粉、肠粉这类都算"快碳水"。不是不能吃,是要减量、加蛋白和蔬菜、别空口吃。

1. 90 天量是省钱的关键,但要提前问。

零售药房一次通常只给 30 天,90 天往往要走邮寄药房(mail order),价格常常更低。这需要时间,所以在还剩一周药的时候就该去续,别等瓶底朝天。

2. prior authorization 不是拒赔,是"要个说明"。

保险公司要求医生递一份材料说明为什么这个药必须用。药房收到通知通常会告诉你,你要做的是打给医生办公室催一句:The pharmacy said it needs prior authorization — can you send that in? 拖着的往往是文件,不是审批本身。

3. 仿制药不是"便宜版的药"。

仿制药(generic)和原厂药有效成分、剂量、效果都一样,差别是厂家、形状和颜色。药房换厂家是常事,不用慌;真的不放心就问 Is this the same medicine?,药师会解释。

4. 漏服原则要按药分开记。

发音与语调提示

1. 数字要一个一个说清楚。

142 over 88 读作 one forty-two over eighty-eight,中间的 over 不能吞掉,它是分子分母的分隔词。同理 130 over 80。血压数字说错,医生会照着错的记。

2. A1c 读作 “A-one-C”。

不要念成"艾克"或者一个个字母快速带过。它是糖尿病复诊里最重要的一个词,说清楚医生才知道你在问什么。

3. metformin 的重音在第二音节。

met-FOR-min。药名念不准,药师会让你重复一遍,耽误的是自己的时间。拿到药名先跟着药瓶上的拼读念一遍。

4. “低于 70” 要说成 under seventy 或者 below seventy。

below 和 under 在这种场合可以互换,但数字前面不要加 about——低血糖的临界值是硬标准,69 就是低,70 就是不高,含糊反而容易误判。
