在美国,过敏和哮喘是两种最常见的慢性病,也是最容易被华人家庭低估的两种。过敏性鼻炎顶多让人难受,但食物过敏和哮喘是真的会死人——区别往往只在于那几分钟里有没有人做对事。这一篇按真实顺序拆成五个场景:查过敏原、食物过敏与肾上腺素笔、哮喘发作时求助、复诊调整控制药、学校和公司的应急登记。

先分清三种情况该去哪儿,这一步错了,后面全错。

过敏原检测这件事,最容易栽在"没停药"上。 皮肤点刺(skin prick test)查的是你身体对过敏原的真实反应,所以检测前必须停抗组胺药:Zyrtec、Allegra 这类要停 3–5 天,Benadryl 停 24 小时。停了才有意义——否则结果一片阴性,白挨一顿划。检测本身很轻:前臂滴一滴过敏原、轻轻划一下,等 15–20 分钟看红肿(wheal)大小。实在停不了药,还有抽血查特异性 IgE 的版本。

症状词也要选对。sneezing、runny nose、stuffy nose、itchy, watery eyes 是鼻炎;hives、rash、swelling 是皮肤反应;wheezing(呼吸时的哨音)和 tightness in my chest(胸口发紧)是气道出问题。这两个词说出口,医生立刻知道该往哪个方向查。

儿童和成人的应急安排是另一门功课:学校要 medication authorization form(家长和医生都签字)才能给用药,大一点的孩子想自己带吸入器还要 self-carry form(各州规定不同);想做成正式安排,可以申请 504 plan。公司那边则是告诉 HR 和主管、登记紧急联系人、把药放在工位,而不是锁在车里。

口语小注

1. positive 在这里不是"积极",是"阳性"。

You're positive for cat 的意思是"你对猫过敏",不是"你猫很好"。过敏原检测里 positive 就是查出来了,negative 就是没查出来。听到别慌,也别理解反了。

2. 停药天数记不住,就记一句 five days before the test。

护士一句 You'll need to stop them five days before the test 已经把规矩说全了:Zyrtec、Allegra 这类 3–5 天,Benadryl 24 小时。不确定就把药盒拍照给护士看,比自己猜安全。

3. It's been three years 比 I have three years 自然得多。

说症状持续多久,用现在完成时:It's been three years. / I've had this for a week. 别用 I have ... since ...,那是典型的中式说法。

4. 鼻喷激素被停掉,多半是因为"没感觉"。

很多人喷两天没效果就扔了。医生的原话是 It takes a week or two of daily use——它是预防药,不是救急药,用对了能把整个春天救回来。

口语小注

1. 打完笔去急诊,不是"太夸张”,是标准流程。

很多人打完好转就不想去了。医生的原话是 always go to the ER — even if you feel better on the way。双相反应会在几小时后杀个回马枪,而在医院里复发和在高速上复发,是完全不同的两件事。

2. cross-contact 和 cross-contamination 在这个场景里都有人用。

问服务员就说 Is there any cross-contact in the kitchen?——重点是"同一口锅、同一块砧板、同一锅油"。只问 Does it have peanuts? 远远不够,因为答案往往是"配方里没有"。

3. may contain 和 made in a facility with 都要当回事。

这是美国标签上的两种警示语,意思是"生产线上有花生"。过敏专科医生的建议是每一次都看,因为同一个牌子换个批次、换个工厂都可能变。

4. 有效期这件事,靠记是记不住的。

五项一次说完,护士不用一句一句挤牙膏。哮喘发作的时候说话本来就费劲,一次说清比来回来去省力得多。

口语小注

1. I can't catch my breath 是最好用的一句求救。

比 I have difficulty breathing 自然得多,也比 I am uncomfortable 有用得多。在办公室、在健身房、在地铁上都能用,任何美国人一听就知道事情严重。

2. two rounds 是急救的分界线,不是"再用点试试"。

一轮 = 两喷。两轮之后还不行,就是 911。这条线在美国的哮喘行动计划里是固定的,不是护士个人严格。

3. 储雾罐(spacer)不是可有可无的配件。

直接对口喷,大部分药会打在舌头上;接上储雾罐,药才真正进肺。孩子和发作时手抖的大人,尤其需要它。

4. 一周两次是"红线",不是"参考值"。

More than twice a week 意味着气道一直在发炎。这个数字是医生用来判断要不要上控制药的,自己也该拿它当体检指标。

口语小注

1. start you on 是"给你开始用某种药",不是"启动你"。

医生说 Let's start you on a daily controller,就是给你加上一个长期药。听到 start、switch you to、add on 这几个词,都是在调方案。

2. 复诊时最值钱的回答是"频率",不是"难受"。

Four times a week 比 pretty often 有用一百倍。医生调不调药,基本就看这个数字:一周两次以内算控制住,超过就加药。

3. rinse and spit 这四个字真的不能省。

吸入激素留在嘴里会养出真菌,表现是嗓子疼、嘴里有白斑。用完立刻漱口、吐掉,别咽下去——这一步做对,副作用基本可以避免。

1. 504 plan 不是特殊教育班,是一份正式的保障文件。

它来自美国的一项法律,作用是把你孩子的需求写成学校必须执行的条款:药放哪儿、谁受过培训、午餐怎么坐。没有它,全靠老师和护士的个人好意;有了它,换人也不变。

2. medication authorization 和 self-carry 是两张不同的表。

1. allergic 的重音在第二音节。
